ACKNOWLEDGMENT AND RECEIPT EMPLOYEE HANDBOOK
(with provisions of Code of Conduct and HIPAA Privacy Rule)

I hereby acknowledge reading the Online Employee Handbook with provisions of Code of Conduct and HIPAA Privacy Rule from Praising Hands, LLC. I understand and agree that it is my responsibility to read and comply with the policies and guidelines in the handbook, I promise to do so. I also understand that if I ever have a question about the policies stated in this Handbook or do not understand them, I should raise that question or concern with my manager or with Human Resources.

I understand that the handbook and all other written and oral materials provided to me are intended for informational purposes only except of the HIPAA provisions. Neither it, Company practices, nor other communications create an employment contract or term. I understand that the policies, guidelines, and benefits, in the handbook and code of conduct, and those communicated to me in any other fashion, are subject to interpretation, review, and change by management at any time without notice.

I understand that Praising Hands LLC management reserves the right to modify, revoke, suspend, change, or terminate any or all such policies and provisions, in whole or in part, with or without notice.

I also understand that employment by Praising Hands LLC is one of employment at will and is terminable at the will of either Praising Hands, LLC, or myself at any time, and that no representative of the Company other than the Executive Director and Administrator has any authority to make any contrary agreement.

TERMS OF ACCEPTANCE and SIGNATURE

I understand that entering my name in the 'Employee's Signature' field constitutes a legal signature confirming that I acknowledge and agree to the above Terms.
